The patient comes in having drank a ton every day for a long time and boom they full blown alcoholic hepititis. Do you use a Maddrey Score or a MELD score that debate could go on for year and likely it's six one way half a dozen another. However, the real question is do the patients really benefit from steroids regardless of which score you use.
So what will the updated Cochrane review show? Likely something like, "Unless the patient is really really really sick there is likely no role for the use of prednisolone and even then it only prevents death at 30 days not at 90 day or 1 year." I guess that's what I would say if I was in control.
